    From the convenience of your own device, you can quickly and easily confirm your appointment and update or verify your information in our system.​ Text messages for pre-registration come from 217.960.9045.     After you have pre-registered

    After you have pre-registered, check in with our front desk staff at your appointment and confirm your date of birth so we know you’ve arrived.​

    Note: Your provider may have additional questions for you at check-in.​

    Frequently Asked Questions About Electronic Pre-Registration 

    Springfield Clinic has implemented a secure online registration option that invites patients to fill confirm their appointment and complete required pre-registration information in a HIPAA compliant platform from the privacy of their personal devices prior to their clinic visit. Patients will have the option to update and confirm their demographic information, upload insurance information, complete required consent forms and for some departments they may be invited to complete basic health forms and other information.

    Patients who fill out their information prior to their appointment will not have to repeat this information at the front desk upon arriving. When patients arrive after completing their registration materials ahead of time, they still need to check-in at the front desk and confirm their name and date of birth to validate their arrival. Our patient access specialist will then confirm your arrival in our system.

    The online registration process is available to all patients with an accurate mobile phone number or email address in our system.

    No problem! If you have a valid email address filed with us, you will be able to fill out the information on a computer.  If you don’t have access to a computer or have an email address, you can still complete check-in at our registration desk in person. Please plan to arrive 15 minutes ahead of your appointment time to do so.

    Yes, Springfield Clinic only uses HIPAA-compliant platforms, and the secure links we use to send your information are confidential and secure.  Patients and caregivers will enter the name and date of birth of the patient being seen at the designated appointment to access these online forms.

    Starting and stopping your pre-visit health forms is easy! Your unique patient information will save automatically when you close your browser or open tab. When you access your personalized link through the text or email you received again, you can pick up right where you left off and see the progress bar at the top.   

    Springfield Clinic currently uses different technology platforms to communicate with you and all of them are HIPAA complaint. The phone numbers will vary.

    Press Ganey and Podium manage their own subscription lists, so if you choose to opt-out of future invites from either platform, please reply STOP to their invitations or click on the unsubscribe link in their email invitations.

    No. Athenahealth provides access to your patient information, appointments, medical history and billing statements in one platform. ​

    If you are a current patient, you will be prompted to verify or update your information that is on file in our system. The link in the text message or the email that you received may ask you to verify or update the following information: 

    • Name (and if you’re filling this out on behalf of yourself or another person) 

    • Date of birth 

    • Medical and family history

    • Address 

    • Phone 

    • Email 

    • Contact preference 

    • Employer’s name
