The Springfield Clinic Skilled Nursing Facility program offers wellness and sick care visits at several local assisted living facilities in the area. Our board-certified adult-gerontology primary care providers care for Springfield Clinic patients under the guidance of their primary care physician. Appointments can be made by speaking with a nurse or manager at the facility where patients reside. Same-day and next-day visits are available as needed.
